Create a PHP/MYSQL/AJAX/JSON web based incident reporting application that can be placed on our web host

Build a web based incident reporting system that can be accessed via secure login from our company website. The system will be purely web based with a mysql / mariaDB backend. We have different clients and as we grow the clients will be addded to the system. Each client will have a number of sites tat we manage for them. Each site will have a number of departments. There will be different types of report that can be input - Accident, Near Hit and Non Conformance but I want the option of adding different types of report in the future so an easy way of implementing that via an admin type panel. Each user will have different rights. Users will be able to create a new report and view reports from all departments within their site of a particular client. Managers will be able to create, read, update reports from all departments at their site of a particular client. Admin will be able to create, read, update and delete reports from all departments, sites and clients. Super admin (only one) will have same rights as admin but also be able to create, ban and delete new admins or users. A dashboard to show how many accidents / near hits / non conformance by client / site / department. Reporting tool to be able to view all open (non closed out) reports selectable by client / site / department (or all departments) - for admin The database will be mysql and it will be a relational model with many different tables. The report will consist of dates, times, environmental conditions (wind, sea and visibility), personal injuries sustained (could be many per incident), property damage sustained (could be many different ones per incident), description of the incident with root and immediate causes, immediate actions to bring under control and close out actions by the management. A log in for the client so that they can view the incidents that have occurred on their site.
