Develop a giveaway application for a retail environment [URGENT]

Create a giveaway application for use in a retail environment. This application will not be on the App store, but an IAP to be loaded onto our iPad's. Our store will be giving away prizes every week. We will be installing iPad's at checkout counters for customers to sign up for the giveaways. Customer will need to enter their Name, Phone #, and Address (requirements subject to change) to enter the giveaway. The address field will use Google Map APIs to show suggested address options and only valid locations will be accepted. All information will be stored into a private DB. Some parts of the entries will need to be cross checked within the database upon entry, to prevent double entries from the same person within a campaign. The database should be created in a very organized fashion, so we can look through information that we've gathered. We will need some sort of administrative panel to be able to create new campaigns. We will want to have an administrative panel where we can create a new "campaign" at will, and roll a random winner when we want to. The application on the iPad could also have a main menu where we can access the admin panel or choose a campaign to display (for use on the installed kiosk ipad's). The campaign display for customers will be very simple. Our company logo, 3 fields to input their data, and a enter button for them to be entered into the giveaway. Our administrative panel will need to have the ability to create a new campaign, edit ongoing campaigns, and role winners for ongoing campaigns. This project is very simple but needs to be done urgently. We are working on a time constraint, we will need to have a working system by April 15, 2018. I tried to include what I think the relevant skills would be for this project, and setup this proposal based on my experience, if you think something needs to be adjusted, we're completely open to ideas. We can rent a vps to store the core system and database, this will be set up upon the freelancer's discretion.
