A small Face Book Quiz Application

I want to create a general quiz for facebook. Please apply only and only if you have prior experience of creating applications for Facebook. The things I need are - the ability to organize questions in categories/sub-categories. - the ability to add/delete as many questions as I need. - the ability to ask as many random questions as I want from the user. - Ability to display a custom message against the passing score. i.e., correct answers are less than 10% display "message 1" correct answers are 10-20% Display "message 2" and so on so forth. - the ability to set these percentage brackets myself and as many as I want. - the ability to add the custom message against each percentage bracket. - A very easy to use interface for the canvas page and profile block with ease to add standard banners. - a "How To" to install it to the face book.
