Create flash from video file

Hello, we need a video file converted to flash, and cropped so that it has rounded corners and can be embedded into a page on a website. We have attached the original video file, a blank psd template (banner_template) and also a psd (banner_template_wheel) which shows how the flash will need to be cropped and where it will go on the page. The outputted flash file needs to be the same quality as the original video file. Any questions, don't hesitate to ask.
