Android App - Not for APP Store

Looking for someone to build a custom app for our tablets. The software will not be in the APP store...the software should be compiled into an apk ready to install. Software will be used for customers entering in their email address...birthday...and phone number...If possible...I would like on the email for there to be 5 buttons for the most common email addresses to save on time (such as gmail, hotmail, yahoo, aol, and so forth). The software needs to work without an internet connection. Creating some type of file on the internal memory would be perfect. So...we hand tablet to customer...they enter in their email, phone & bday in a very easy and fast method...it says thank you...and resets for new customer. Later we access the tablet and retrieve the email file. Software must work with Android 1.6+
