CUSTOM JOOMLA 1.5 LOGIN MODULE

I want a custom login module for my joomla page. When someone clicks the Register button these areas should be obligatory to fill. Name-Surname Username( Needs to bse checked if someone else has the same username) E-Mail (Need confirmation) Password Captcha And the registration needs admin approval After the registration when the registered user clicks on 'Edit My Details' link these areas should be ready for editing Name Surname: Text Area Password: Text Area E-Mail: Text Area GSM Phone: Home Phone: Ofiice Phone: Fax Number: Gender: Checkbox Male or Female Company: Text Area Profession: Text Area Place of Birth: Text Area Date of Birth: should be edited in dd/mm/yyyy ID Card Number: Text Area Press Card: Check Box Yes or No. If yes ask for the number in text area Blood Type: Radio button for all types Marital Status: Check Box Single-Married Children: Check Box Yes or No. If yes ask for number in text area 5 more optional fields that can be edited by admin. All fields titles should be edited by admin. (Cause i'll translate them in Turkish. So the codes must be suitable for Turkish characters)
