Basic Poll Creator Software

I need a simple piece of software where the user can enter a few variables for a basic survey, and then the software gives the HTML code for the poll to be placed on a site. The user can then copy and paste the HTML code to their site so that the poll form is ready made to be used. When someone does the poll and selects an option and submits it, the results should be entered somewhere so that the creator of the poll can look at them. Just as long as it works and the person can easily access the results is all that matters, I don't care much how it's done just as long as it works and is as easy as possible for anyone to install. The software should be .exe and I want the focus to be on ease of use with creating the survey and looking at the results. Try to keep it as simple & easy to use as possible. As an example of what the software should look like & an example of the type of poll I want it to create, see the attached file. I am looking for a good reliable programmer to develop a few other scripts I need in future, and so the winning bidder should get several more projects in future if the project goes well. I really need a reliable programmer to develop a few other related scripts for my membership site and, and as I don't yet have a programmer, this project is partly just to find someone good who can develop some simple software ideas I have. In fact, the winning bidder is almost guaranteed to get more work from me in future if they are available to develop a few other scripts I need after this. *I will retain all right to ownership & distribution of the script. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). ## Platform .exe file for the software that generates the HTML code for the poll. Any other scripts needed.
