79438 Simple ASP form

I have a simple html form that needs to be converted to ASP. This form currrently collects first name, last name and email address. It needs to also collect an affiliate ID from the referring URL and pass that ID into an email along with the other data. There is some documentation for doing this at my ISP: [login to view URL] This should be a very simple project for a programmer than knows what they are doing.
