Simple form with repeating items

I need a simple submit form with several fields (about 8 fields in total), the final 3 fields need to be repeated until the user has entered all the items, e.g. they complete the usual name, company address fields etc at the top of the form, then the last question in the form they must specify their choice of items (number of items, weight, and dimensions W/D/H) They are able to add as many items to the submission as they like, so would need some way of expanding the form if they want to keep adding items, or a "add item and specify another" button as well as the usual submit button etc. We dont want a database, but have ASP available, perhaps this is a cookie task. Open to guidance anyway....... Thanks
