Grocery Delivery appointment booking

This project is coming to an end but I have the last part of that widget that I need to get completed. I basically need a scheduling / reservation widget that is in the final checkout portion of the cart that is broken into 2 hour blocks. It is divided into 2 different groups and here are the locations. I will need to add routes or locations as the company grows. Group 1 Alachua 32615, 32643 Jonesville Location 32669, 32606, 32653 Group 2 Gainesville location 1 32601, 32603 Gainesville location 2 32608 All timed orders for next day deliveries, weekly, and monthly can be time as early as 9:00am as long as it is placed in advance. This is not available for same day deliveries. Same day deliveries are set up in a time slot with a two hour time block for deliveries. I would need to keep this on my server since the client is currently looking and testing out the site. I need this completed in a timely manor.
