Template customization... #2 (Flash Header)

Please view the attached file so you know exactly what I need. Do not stary from the design. Do not try and type your own words beneath the logo. I will award the job to the person who meets the following criteria: 1. Must speak English. 2. Must have a form of IM and begin work IMMEDIATELY 3. Must be able to reproduce the final product demonstrated in the last link in the document. 4. Must quote a fiar and reasonable price. I have laid everything out for you of what I need, don't charge me for excess stuff. 5. Must be willing to code the header in the future once links are mapped. I am looking on the low end of the spectrum here. There WILL be more work for the designer in the future, especially if you know how to code... Please do not send me copy and pasted text, let me know you actually care about the project by taking enough time to type something unique.
