Integrate PHP form with HTML page, files will be provided. Need completed in 30 mins

We have a single html page which we need to integrate with our php form. We will provide all contact form scripts and php files. the code just needs to be renamed to make sure it works with the text field. And we also need to edit the formatting and hyperlink it should take about 15 mins to 30 mins to complete as we're providing you with pretty much everything. Hence the very small [login to view URL] are looking to spend $10-15. This will just involve renaming the text boxes so they work with the POST function on the php files and adding the submit function to the button on the HTML page.
