URGENT 1 Hour: Show series of images (gif?) / then webform

I need a simple web page that shows a series of 10 images all the same size (could be an animated gif, or a javascript gallery) showing each for 8 seconds. After the images have been displayed a webform is displayed and the user can fill it in and press submit. They are then shown a thank you page with links. I need this in ONE HOUR! It's really easy for the right person, please show me you can do it. The animated gif / gallery should be 922 width x 394 height.
