Build a Django template with javascript

I have the Django web application running on my system (development environment). It is a performance evaluation form for my employees. The form consists of questions whose with choices and each choice has a score. Need help with building the template where the scores are displayed(A kind of a pop up) dynamically as user selects an option for a question AND total score is computed depending on which option(choice) user selects at the end of the form. When user hits submit after answering all questions, the form is stored as a row in the database with final score. I have already made all database connections and project is completely running. Just need help with this particular template and score computations and displaying it.
