Quiz application platform

Quiz application with about 25 parts each with reading then a quiz. Before taking the quiz about 10 fields of information needs to be collected (first name, last name, email address, and so on) This is for a traffic ticket school Flow 1. Choose your city (list of cities that we provide, in California) 2. Choose your court (depents upon the city you selected, we will provide this info) 3. Enter your information (about 10 fields to collect from student) 4. Takes reading + quizes. 5. For payment and final test we would like to post all of this information to a website and have the person finish outside of the application. We will do this website I found an example application ([login to view URL]) that could probably be modified to do what we need or you can create your own application. The only issue that I see is that for the quiz for one county (Los Angeles) needs to be timed to 400 minutes split evenly amongst each reading. The rest of the counties do not have any timed reading, the user can go through the course as fast as they want. We would need any code created so that we can make modifications in the future.
