Iphone app interface for cakephp website

I need someone to program an already designed iphone app. The app is based on a website built using cakephp. The app will pull from and submit data to the mysql database used for my website. The interface and functionality is already established and I can provide mockups of that. The basic concept is that it is a appointment scheduling app. The iphone interface will submit data through forms to the web application which will produce the same results as if the person were using the web app. The iphone interface will then pull the information produced by the web app and display it in the iphone interface. The app needs to be able to check periodically for new information regardless of the person opening the app. The app will need to be able to send notifications to the user depending on the status of the appointment. The iphone interface will mostly be made up of forms, but the design has boxes for each of the people you are trying to establish an appointment with. when you click on different icons on the box the box opens up (accordion style) to display the information you are looking for based on the icon you clicked on. Basically if you can convert a cakephp website interface into an iphone application, that is what I need. The web app will not know a difference between who is using the website and who is using the iphone application. The information that the app will be pulling from the database will be very specific and will take some significant filtering to get the right information but you will know exactly what to pull by looking at the code on my website. You can see according to the screenshot I attached the top box is a opened box and the bottom is closed, any person can have any number of boxes and they can scroll through them. On the bottom box you can see that the appointment status is shown on the bottom left, the info on the bottom right will be different depending on the status. The top box shows what will happen if they click on the name. They can then choose one of the three action icons to the left which opens something. The example shows if they were to click on the info icon, if they click the email icon a form pulls up for them to send that person an email. This example shows the families link at the bottom, the other two (scheduling and settings) are just forms that submit back to the database.
