106015 Survey Generator Widget/Script

I am looking for someone to design a script to generate custom surveys that will store the info in a mysql db and allow people to post these surveys on other sites and submit the results back to my site. Users would be able to visit a link to see all of the responses to the survey. An example of this script that would be ideal is: [login to view URL] This module would not necessarily need to have a user system. It would be acceptable to have the survey generated and added to the db and not allow it to be edited.. A token/id would reference the survey for posting results and viewing the results. If you have something like this already and/or feel you are capable of creating this please send any examples and proposals in a PM.
