convert from asp to php/integrate w/salesforce/ ip based dynamic content

We need are going to migrate the site to a new LAMP machine. We need the asp converted to php, and a cms built to let the users control the page order, links, and content. We need the initial "lead capture" form to submit data to our salesforce/insidesales (they integrate) crm AND to post to a Mysql database as well. The lead capture form goes on to provide a test for the users to take and it 'grades' the test (the test has three sections) we need the scores and the lead info posted to the mysql db AND the salesforce crm, ammending the initial lead capture. This site is now currently serving the Raleigh NC area, but we are expanding to dallas TX, and we want the feel (headlines, address, etc) to be controlled by the viewers IP address. So if a visitor was from a TX IP address the headline would read "The best in Dallas for..." the location address would read: "123 main st dallas, TX 12541" and when the user was from a NC IP Address we would deliver a 'Raleigh' based headline, etc. We also have a thank you page that is the last page of the "test" that our opt in leads take, and on this we ask them how they heard about us. They enter their zipcode, and other info before these options are given for them to choose. We want to use either AJAX or IP delievery to make sure that they are ONLY offered releveant choices. For example we are on 20 radio stations in Raleigh and 20 in dallas. If a user is from Dallas we want to ONLY offer them the Dallas radio choices, and in NC we want to only offer the NC radio stations for them to choose.
