php mysql ajax form generator class

Hi all, I am Lookign for a php class which allows me to create php forms with a click of a button once the form has ben completed create a mysql query of all the html for a sperfic page and aswell as php code if there is any on a button which can be stored in the database so it can be changed later so basically wanting a single page php class that allows me to create forms and allows me? to have a popup menu and asks me if i want it to update a certain table connecting to a current database basically aks a user on how many table fields they want to create depending on how many form fields that have been created taking the name of them and adding them on to the end of the database table structure. this is for a gaming script im developing atm and no sure where to start if this can be done in somthing else other than ajax that would be great. basically you just going to be showing me and teaching me the basics on how to achieve somthing like this. Any help would be good not sure on how much this would cost and can you give me a qoute? Thank,William
