Web App featuring Google Maps API with Driving Directions

Hi guys, I'm working on a web application and I need some help. What I need is a form with two fields, one field for the users current "location" and a second field will be for the users to enter their desired "destination". Next to the form, I want to have a google map. Once the user fills in the fields and submits the form, the google map will update and show markers of the users location and destination. I will also need the "miles" between the location and destination to be returned, so that I can do a calculations with it later in PHP.
