Modify some items on my site

I am in need of a couple things modify on my site [login to view URL] , if you view the attachments it will explain all ... if you go to [login to view URL] and click on any city or state, then click on to escorts then the gallery, and run your mouth across any image in the gallery you will see that the user images rotate ... if you have any question please feel free to ask me ... now its something I did not put on the screen shots, my signup form the category are missing , I will like that issue fix to [login to view URL]
