154503 Order form and client area

I'm looking for a simple order form similar to this [login to view URL] customer must be able to pay via paypal. Client area will reflect information on the order form and enable customer to edit their details and view their hosting login information. A simple admin panel would also be good so I can view/edit customers etc. I've been told that this is not a big job so if people are going to quote megabucks I will get an off the shelf package. Thanks
