** Simple php script **

Need a simple php (mysql backended) script which will recreate the following URL: <[login to view URL]> Admin Area: There needs to be a password protected admin script where the following fields will be asked from the admin: Store Name: Address: City: State: (drop down with state abbreviations) Zip code: phone number: County: (drop down with 3 counties: Dade County, Broward County, Palm Beach County) The admin should also be able to list any and all stores by city (drop down menu), zip code, or county and delete any stores which need to be deleted. USER AREA: The main site URL: <[login to view URL]> Should have a drop down box which shows a choice of every city which has been submitted and allow the end-user to choose a city and see a list of all stores for that city. End users should also be able to enter in a zip code for search. End users should also be able to drop down a county choice and see all results within that county. Only one search choice for the end-user (not multiple criteria). The main page must be recreated with all of the listings shown which can easily displayed on one page. Possibly second page would be clicked on from a next link for more stores. the page must look like it currently does. Final code must be fully implemented on my server. Will give ftp and mySQL db info to facilitiate. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables): a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment. b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request. 3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement). 4.) Coder understands and agrees that this is a small project and partial payment shall not be expected for partial work completed.
