Change Control Form - Perl, JavaScript, IIS

I developed a school project in Perl, JavaScript and MySQL. I would like the database to be moved to MS Access 2000. I would also like it cleaned up and made more professional. Currently the menu choices are driven from static text within the Javascript. I would like them changed to drop down box which is linked to MS access datatbase. I can provide all current code for enhancement. I would also like a more robust search feature as well as a confirmation screen and the ability to modify (i already provide a screen with confirmation of data, but it does not allow you to modify, it just continues to submit). Needs to be fully coommented. ## Deliverables 1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done. 2) Installation package that will install the software (in ready-to-run condition) on the platform(s) specified in this bid request. 3) Exclusive and complete copyrights to all work purchased.
