Clean Html form style...

I need help in changing one html page to a better looking cleaner page. The page is done in aspx but I have provided the htl version of it. The page looks crowded, implemented with a lots of tables... the stylesheet is also not very cleanly done. You need to clean it up so that the page looks more professional (preferably with divs (but not absolutely necessary)). The usability of the page should also improve. Please provide a sample with your bid... the sample does not need to be? exactly the? screen I supplied, you can show me an example on the web or it can even be a photoshop mockup... Note I have two html files here, [login to view URL] becomes [login to view URL] when the create signatur button is clicked. So you should not change the enclosure...
