Code one new PHP page on my existing site (hopnavadmin)

We have an existing PHP site, connected to a MySQL database. The job is as simple as creating one new page and posting data to and from the database. I've even given an example site for how it should work. Please read through the attached spec sheet and feel free to ask any questions you might have. The winning bidder will be given full ftp and cpanel access to complete the job and I expect the work to be tested by you (in multiple browsers) before you tell me it's done.
