Fuzzy Address Matching

I have a mySQL database table containing black-listed addresses. The table is split into fields *address1, address2, city, region, postcode, country, country_iso_code*. I need an efficient and accurate algorithm implemented in php which will fuzzy match a supplied address against those contained in the database and return a probobility score along with the results.
