New page on website with java

Dear Coder, I have a form that I would like to be put on a website. When people have purchased a product from i want them to be sent to this page. I want one page to be set up where people can add their details and answer certain questions. Tick boxes etc, input information. I have all the questions and file ready. Ideally you will have experience designing nice looking forms. and be able to put them into a word press website. As regards functionality, I would like the form to be able to have a save function so they can save where they are at on the form, and return to it later. Ideally i would also like a automatic save function on this page. Thanks
