form/ticket maker(service-repair ticket)

Hi, i want a small software where i can make repair service bon/ticket. .exe (so can be used offline) a) company logo/adress (i must able edit/add these company info) b) customer info : (i must add here customer info - but later i must able chang cutomer info - tel nr etc - so editable) c) item info ( repair service item information )(i must able edit item info also ) d)some check box buttons e) some place to write extra text. i must able in one pace make service ticket. i must able in other pace edit/add list/check box text/ i have example made by someone in msaccess -visual basic project sources codes.. i cant edit/add so if u want u can use these just need fix some bugs (print out - edit text not possibel) a) company logo - company name-address-postal code - city - country - tel nr - email - website b) customer info - Name - address - postal code - city - country - tel nr. - email c) item info - Item (list for select items - admin panel list for add items )-(model - if selected item example nokia - then list of models ) (serial no - i can here enter serial number of repair item )(data delivery - (date of today) - date of pickup (i must able write-select date when i want customer can pickup back)- (Repair info : here place to write problem if repair item ) Check box - Recieved item: battery (check box) - simcard- (check box) - charger(checkbox)-memorycard(checkbox)- other (place to write self if somethink else received extra) List of items : 1) List of items what i want add to these list i must able edit-remove (example Gsm - computer - game console inside list) - 2) (list of early added problems (example list with Touch screen damaged - water damaged - doenst power on - new windows installation..) Price : check box (warranty)(out of warranty) - And place to write Price of repair Down of pace place to write some small text (pickup within max 8 week-all repair 3 monht warranty etc..)
