BreezingForms joomla Help

Hello, I would like someone to help to build professional forms with BreezingForms Joomla I'm looking to build only 2 forms that are identically the same, only a few fields are different The form will be printed in excel for the admin and the user The form will have some fields that require calculations The form will be similar to this one in design: [login to view URL] I'm looking for a user experience form that will help our users to access and upload their info ... etc. Thanks,
