Car parts database search

I need to build a complete website . It is a car parts database search site. It needs to have a [login to view URL] page where visitors to the site can enter text (using HTML <form> and <option>). That search is then passed to the PHP page (using POST). The MySQL table needs to have 5 columns that can each be searched by. The [login to view URL] page should have a <form> and <option> drop down selection so that if a car manufacturer is specified, only that brand/make's parts will be returned(see attached picture). I would like PHP and MySQL queries used as much as possible but some JS is ok too. Also the search page should have the following search functionality: Contain, starts with, exact match [login to view URL] SQL database example: [login to view URL] I dont need the sql database built, I did that already. Some simple styling/ccs is fine, but don't worry too much about it. I would just like the search results formatted nicely, Like a simple table with the search results would be fine. Additionally: If the search result page could have pagination and If the results can be exported to pdf/text/cvs.
