quick fix - complete contact form angular 1.6 and php

i created a page using angular, ui-router, bootstrap, etc the site is an [login to view URL] files it loads "states" the form is in a state i need to add a radio button to the form, it must change the email option for a phone, so the user can change if they want to be contacted by email or by phone then i need angular to send the data of the form to a php file and php file need to be modified so it sends the data to 2 emails the working page must allow user to pick phone or email, submit data and send an email look and feel must remain as similar as is only bid if you can start right now!
