Single page form and image uploader + an admin

Hi, we need a single webpage that will be uploaded to our litespeed (linux) webserver. - It will be used for ordering where many customers will send us information for one large production. Front end: Form fields (Normally 3-4 fields) Image field (User must be able to upload image file, and resize/corp to size 3x4cm) (Uploaded images must be stored in a folder on webserver with unique image names) Back end Add/ remove/ customize front end form fields Sort form data by date/ one ID-field Export as excel after filter (this will generate a downloadable excel file where all filtered forms is listed. Its important that the image file-name is also included in the excel sheet.) We need the front end to have a professional look, and form fields must be aligned and look nice. - Also the uploader must be highly stabile and userfriendly - It must show status bar and things like that so user dont get feeling things are "stuck" when uploading file. - Auto scale file to fit window and let user adjust scaling and position in a rectangle that illustrate the corp area. - need to work on all browers and accapt all majore image files. We would like to see a screenshot of a image uploader that you are planning to use before we award you. I hope someone are confident that they can make a good solution for us
