Ipad and Android Tablet App -- 2

Hello k2appsolutions This is a very simple task. Look at the picture #1. This is how it should start. its a 7 page PDF slider with only one button, which says "click for patient chart"(Pic 3) Whatever the page is they click and it goes to a contact form. They fill the form and they should have the ability to send it to the electronic address that they want to send it to. that's it. Client should be able to list through the pdf like a slider. Look at the pictures and let me know how much for Iphone and Android app. I know there is really not much involved, so please be very considerate.
