Feedback website using Wordpress

Looking to create a feedback website for small businesses, eg. Cafes, Restaurants. So customers can leave feedback for the business owner. The idea is to replace the traditional pen and paper feedback form with a web based solution. The business will pay for the solution/service. I want 3 pricing options: 1) Free (customers submit feedback, which is sent directly to the customer via email) 2) Paid-Lv1 (same as Free, but comes with customised feedback page, ie. logo/colour scheme; once-off setup price) 3) Paid-Lv2 (same as Lv1, but includes membership and access to reports, forums, and past feedback; subscription price) Using Wordpress and free plugins, I can easily build options 1) and 2), but 3) is little bit trickier for me. I need somebody who can build 3). If you can build 3), then I'm sure you can integrate 1) and 2). I haven't shown it on the attachment, but I would like to have a search function on my homepage, to allow people to search for Businesses so they can give them feedback.
