Wordpress plugging slider tabs

I am looking for a Wordpress plugin to be developed that enables several bright tabs on the right of a word press site. These tabs should be clickable and should then slide a box from right to left with the relevant details. For example one tab could be contact us. You when you click on the tab a box slides from the right with a mall form to submit. When you hit submit it slides back.. Another tab could be find us, when you click on the tab it slide a box with google maps location. The tool needs to be very simple and easy to use and you should be able to set up multiple tabs. Some tabs I should be able to link to a page such as Facebook or titter. Also need to be able customise colour of tabs. Can all applicant provide experience in this field with example of work. Important Details about the script. i. I own the source code to this project and all copyright ii. Code should be well commented iii. Be available for future updates and support (if needed) iv.
