Medical Note Writing

Our clinic uses a Spreadsheet note for documenting progress of clients. This is a month at a time spreadsheet with different "pages" for input. I would like to get a bid on making this note system as an IOS App and a separate bid as computer software You can see a screenshot of these notes at: [[login to view URL] A][1]s you can see at this link there are many little cells that we put information into. I would like to have a view of the entire month with these cells opening up into a larger window to input information. I would also like to have the ability to "attach" a photo or short video to any cell. Would also like to possibly be able to add a way to record audio to a specific cell or date. I like the Notes Plus Ipad app feature on adding audio. Each note would be client specific so it would need to be able to be saved to create multiple client notes for multiple months so Dropbox or similar access would be necessary. Essentially when we write a note we have to have Day, time, treating provider, client, task worked on, score on the task and a place to write a narrative note.
